SYNTHPOP GOES THE WORLD - 3-day ticket order form

This form is ONLY to be used if you are paying by certified cheque or money order
(NO personal cheques will be accepted). To order tickets, print out and complete
this form, include a certified cheque or money order, and mail to:

All certified cheques or money orders must be made payable to DJ Lazarus. 

Once your certified cheque or money order has been cleared by our bank, you will
receive an email or phone call to confirm your ticket purchase. 
*NO TICKETS WILL BE MAILED OUT.* 
You must bring your ID to the ticket booth at the venue to receive your official
plastic 3-day pass.  No mail orders will be processed 2 days prior to the concert
date or thereafter.  Kindly allow at least two weeks for mail delivery.  Late
payments will be returned to sender.
